Communication's Role in Creating a Culture of Safety

We think of culture of safety as a relatively new term, but it did not originate in healthcare. It originated in response to the Chernobyl catastrophe in 1986. Warnings of things to come had gone unheeded. Had a formal safety culture been optimized, the outcome might have been less tragic.
