Associate Membership Application

Associate Membership provides a cost-effective solution for department supervisors and those on the practice management team as an add-on for practices with one or more essential or premier ASOA members. Associate members receive e-access to Administrative Eyecare magazine, and member-rates for ASOA webinars and annual meeting registration, in addition to other benefits. 

Please complete the information below to establish an Associate membership for an employee in your practice. An ASOA staff member will contact you directly regarding membership dues payment methods. 

Associate memberships must have a current Premier or Essential member sponsor to be accepted.
